“The Hospital Construction Act: An Evaluation of the Hill-Burton Program” by Judith R. and Lester B. Lave analyzes the 25-year record and the current utility of this popular program. The programs conclude that Hill-Burton has fulfilled the goals of the original legislation. They recommend its termination.
Judith and Lester Lave find no overall shortage of hospital beds in the United States today, despite overuse of inpatient facilities. The major problem now, according to the authors, is not how to provide more beds, but rather how to control hospital costs under third-party reimbursement practices. Provision of more new facilities will only worsen the problem of cost control by causing both lower occupancy rates and more extensive overutilization of hospital services. For these and other reasons, federal subsidies for construction are no longer needed and may indeed be counterproductive, except in the case of hospitals with a high proportion of nonpaying patients. The authors advocate that third-party payment systems be redesigned to enable efficiently administered hospitals to meet their capital needs and to expand. In their view, an incentive reimbursement formula would motivate hospitals to improve efficiency and reduce redundant facilities.
Judith R. Lave, associate professor of economics and urban affairs, and Lester B. Lave, department chairman and professor of economics, are affiliated with the Graduate School of Industrial Administration at Carnegie-Mellon University and are experts in the economics of health. Their recent work include investigations of hospital cost functions, health care delivery systems, specific medical care projects, and incentive-reimbursement plans for hospitals. |
