Ginsburg, concludes that consumers must make out-of-pocket payments for medical care if a private, decentralized system of medicine is to be preserved. Complete coverage of all medical expenses for the entire population, they argue, would lead to an explosive rise in costs, and any attempt at regulation would probably result in the “nonprice rationing” of medical care—people unable to find a doctor to serve them and extensive waits for many medical procedures. As a viable alternative to otherwise necessary cost sharing, Frech and Ginsburg suggest that health maintenance organizations, such as prepaid group practices, have the potential to provide competitive, nonprice rationing of health care that is sensitive to consumer preferences. They believe these organizations offer important advantages for any national health insurance program. H. E. Frech III is an associate professor of economics at the University of California at Santa Barbara. Paul B. Ginsburg is an associate professor of policy sciences and of community health sciences at Duke University and director of Duke’s Center for the Study of Health Policy.
