The repeal of the ACA and its implications for Medicare spending, patients, and physicians

摘要Prepared remarks Thank you for having me here today. I am a general surgeon so I don’t often get to talk to a roomful of ophthalmologists. One of my favorite fourth year medical school electives was an Ophthalmology rotation at Manhattan Eye and Ear. A big part of why it was so good was a resident who left general surgery residency after 2 years and moved into ophthalmology or as he described it, moved from being miserable to being happy. I often rue my failure to follow his example. But why talk about past mistakes when we can talk about health policy? Today I want to talk about the future of the Affordable Care Act (ACA) under the new, Trump administration and in particular what can we expect in Medicare. Anyone who has followed the news knows that in approaching this task a large dose of humility is necessary. It’s worth remembering that: “It’s tough to make predictions, especially about the future.” No one really knows the source of this adage but it is clearly applicable to the current healthcare arena. I want to start out by examining what the ACA set out to do and examine its intellectual underpinnings. By the end I hope you will agree that this radical overhaul of the American health system, much of it mediated through the Medicare program, was not justified. Read the full remarks.
