摘要 | Nearly 31 million Medicaid beneficiaries have one or more chronic illnesses and require a variety of health and social services. States interested in identifying more effective models of care for these beneficiaries are looking to various integrated care models, including the Centers for Medicare & Medicaid Services (CMS) Financial Alignment Initiative for Medicare-Medicaid enrollees (“dual eligibles”), to enhance access to services, improve health outcomes, and curb growing health care costs. This brief highlights best practices in disease management/care management programs and considers ways in which states can incorporate them into integrated care models like the Centers for Medicare & Medicaid Services financial alignment initiative for Medicare-Medicaid enrollees and other state initiatives for high-cost, high-need Medicaid beneficiaries. |
