摘要 | People eligible for both Medicare and Medicaid—including low-income seniors and adults with disabilities—have some of the highest levels of need and the highest expenses of all beneficiaries. States are developing integrated programs to better coordinate the full range of medical, behavioral health, and long-term services and supports for these individuals. These programs use a wide range of approaches to make one entity accountable for the full continuum of an individual’s care needs, with the ultimate goal of creating a seamless patient experience. |
